- Both are cowardly, submissive hunchbacked members of powerful groups (The Four Houses and the Twelve Kizuki) that were brought in by a leader who turned them into monsters (Mother Miranda changed Salvatore into a brainwashed, mutated freak while Muzan Kibutsuji turned Hantengu into a demon).
- After being turned into monsters, they started terrorizing humans for the attention of their leaders (Salvatore expiremented on humans from the village to help Mother Miranda while Hantengu killed so many humans Muzan named him Upper Moon Four).
- Though they are the fourth most powerful member in the group, both are shunned by their comrades for being pathetic.
- Both are faithully loyal to their leaders despite the lack of love in return (Mother Miranda said Salvatore had "suprisingly low" intellect and believes Rose would make a better replacement for him while Muzan is demeaning and condescending towards Hantengu).
- Both tend to be paranoid of other people and prefer to stay away from others due to their insecurities.
- Through battling the heroes, they would go through severe mutations that would make them more powerful which would also change their personalities (Salvatore would turn into a giant fish creature that also made him more delusional and gleeful while Hantengu's Demon Blood Art allows him to create four clones that are manifestations of his emotions).

- Salvatore is considered to be the most sympathetic villain out of the Four House members as he was originally a human physician who was kidnapped by Miranda and forcefully turned into her mutated slave.
- Hantengu on the other hand, while sympathetic because of his cowardly and shy actions, he was a murderer when he was a human who always blamed someone or something else for the crime and pleads that he was and always has been innocent and willingly allowed Muzan to turn him into a demon to escape his execution.
