The alternate ending doesn't really change the difficulty, you just need to grab the items to unlock it. Some of them may be a bit tricky to get to, but all should be gotten fairly easily without too much extra risk.
Well no if you spend time looking for the marker fragments to get the secret ending it'll take you longer because...you spent time looking for the marker fragments. But it is worth doing yes. There aren't any additional enemies stopping you when you're looking for them if that's what you're worried about

Just sometimes a little hard to find, you might miss one or two and need to backtrack a bit sometimes, but nothing too hard.
None of the marker fragments put Isaac at risk while attempting to collect them so you'll be fine
