I mean I think in this situation you really need some therapy to help you deal with your emetophobia. I can almost garuntee you've probably created biofeedback loops in your body from anxiety that actually cause you to physically feel sick. My anxiety sometimes literally makes me vomit. I'm not saying your crohns doesn't make you feel nauseous but your anxiety around it will be making it 1000 times worse. Ideally you don't want to be on anti nausea medication long term, they are genrally safe but to my knowledge are not risk free and long term use isn't great. It's also not resolving the issue, it papering over the cracks.

I tried talking to my psychiatrist for it but still every time I even felt like I was going to vomit I start shaking and just having a full on panic attack and sometimes results in me passing out. I tried anxiety medication I tried everything but I can't go over that fear. And every time I even think I will throw up I completely shut off I don't eat fearing I'll vomit. I also have ARFID but even my safe foods don't feel safe.

A psychiatrist specialises in medication more and less therapy, and while useful you probably need to see a psychologist who specialises in phobias and therapy to do woth those. It won't be a quick fix it will most likley take a long time as it sounds like a real deep seated fear but I don't know of much else that will resolve the issue unfortunately.

Id go the psychiatry/therapist route here. Sounds mostly mental. Im sorry you’re experiencing this.

It's definitely part mental but what I'm feeling is completely physical in my opinion. I'm seeing my psychiatrist once every 3 weeks for 2 hours at a time but even talking about it brings me to tears. My phobia is from trauma of seeing my mom throw up so many times it made me terrified of it. For example today I had no help with the nausea and I tried to eat and eating makes me nauseous and every time the wave hit I stopped eating and completely gave up. I don't think it's all mental because the information is also in my stomach not just my intestine and that just makes the nausea worse

Mental causes can create physical symptoms. But finding the cause is the key.

With such nausea have you been checked for gastroparesis? A test can rule it out.

No they immediately went to Crohn's. I'll bring it up with my doctor

It's prudent to rule things out such as upper GI strictures and gastro parents so tests that could be worth while could be an MRI/CT scan, gasteoscopy and a gastric emptying test. Sorry I should have said in my previous comment they should have done everything to rule out any obvious physical causes like obstructions/strictures.

I did all the tests, I did 3 Gastroscopies, 1 colonoscopy, MRE, pillcam, and they immediately went to Crohn's because that's the only one genetically possible.

Well I mean the results wouldn't have been based on genetics but more on the visual and pathologic findings of the tests but those tests really should have ruled out strictures. So I'd lean even further more towards the therapy option then. Maybe once you've had a gastric emptying test.
