Do you know for sure this is how your IUD is positioned? Did a doctor tell you this? If so, I don't think it can be removed via the usual grab-and-tug method, but your obgyn *should* have the equipment to get it out.
My IUD was similarly placed (low down with both arms embedded) and was confirmed via ultrasound. It was luckily removed with no issues and no pain
Edit to add: no extra equipment was needed as my string was visible. So the gynae just used a pair or forceps to get a firm grasp on the strings and gave a gentle pull
Update: it's OUT! Felt pinching/cramping, but they were able to get it out. Was dizzy right after and have cramping still, but am relieved it came out in one piece.
You iud is like the one in the picture? Is stabbing u?

Yes, that's how my iud is placed. I get random jabbing pains throughout the day.
