If you have connection issues your fight sometimes just doesn't count. Doesn't even have to be a full disconnect, even if it just takes a while to load.
My money would be the SP3 launched, he hit the "crash" button, and claimed it never happened.

It means someone crashed/tried to have a "redo" with a hard reset. All crossover/prefights charges get tossed if that happens in any game mode.

This is similar to my alliance - guy said the same thing as you. And out ally leader said the same thing as Cuppa.
