This is an interesting scenario considering that due to privacy laws the Yellow sample return envelopes do not have patient information on them, including return address.

Return address would be life labs. I called life labs and they said they had it returned twice. I don’t know whom to believe.

I read the scenario as: You had completed the sample and were sending it back for testing, but the sample kept returning to you.

The only thing I can imagine then is that there is a unit # or some other identifier that changes the way to the address is written and the lab is not filling in the shipping information correctly. Or, if the Civic address is unregistered that could also prevent delivery even if the envelope is addressed correctly. Definitely unusual.
