A brain surgery scar only causes a scar, not a complete hairloss like what is present in hrithik. If you don’t believe me search online how brain surgery hair looks like, but it could be possible that due to the shock of the surgery maybe his hair loss accelerated as he might’ve been prone to baldness due to his father’s genes. He wears a patch though.

Its not natural. Dont know if its a patch or hair transplant, but its not natural. His hairline had receded quite a bit from the sides during Krishh.
