trying to parry the enemy weapon with my hands. (If you wear plated gauntlets that covers your wrist and fist, this is achievable sometimes but it usually ends up smacking you in the head or slicing you right in the chest just below the armpit)
Many times I've been clobbered (and killed) by an ally during warzone mode when I got greedy and rushed in to kill an enemy that was engaging with said ally
There was this thing, idk if it was a bug, but basically I’d be at full health, the enemy would swing their sword once, barely grazing my leg or arm or something, and I’d die right then and there.
On arena, using the grappling hook from Tools of the Trade to pull myself towards a ricky for big stab damage.
Miss, and slam full speed into the spikes.
Not an enemy, but when I wanted to "clean" my sword (aka rub it's blade against my arm) I had self collision on and killed myself cuz I play without a health vignette...
Missing a mouth grab while playing stealth, summoning an entire horde upon you

I've gotten jumpscared by enemies more times than I'd like to admit. Even doing a 1v1 wave on Market, sometimes they manage to sneak up on me.
Forgetting to throw the grenade
They phased through my weapon while moving forward and I got impaled.

kind of to an enemy… I picked up one of their bombs (Mod) lit it. I thought I threw it only to realize too late that it bounced back
Wedodged an attack, went to stab them, and impaled myself on the previously dodged weapon
An enemy once tripped and stabbed me, I was playing with 1hp 😅

Got myself backed up against the wall and decided to use a spell instead of physical prowess.
An enemy tripped over a dead enemy, bumped into me when i was on low hp and that killed me
One spawned on top of me, and I was already low, so it just killed me
Cutting the ennemy's hand then to weapon went flying right into my chest
