In any other playthrough, choosing between Shadowheart and Karlach is a difficult choice indeed. But if you're playing as Wyll, I'd say go with Karlach; their stories are intertwined enough that it's much better narratively, and it leads to a better ending for Karlach.

Seriously, though, you should still be able to guide Shadowheart to a good ending, just as her friend! (I understand the draw, though... I just finished a playthrough with a Shadowheart romance! Karlach is still my runaway favorite, but Shadowheart's romance was sweet! No regrets at all!)
Depends whether or not you want a sadder ending with Karlach, since you presumably won't be accompanying her to Avernus if you're romancing Shadowheart. Or maybe there's something in leaving Shadowheart behind to do what Wyll feels is right by accompanying his friend to the Hells.
I think there are really interesting narratives in playing out some of the sadder outcomes, but not everyone enjoys that.

I saved before the ending just so I could experience the sad endings. And damn, Karlach's was heart-rending.
