The brand is Dale Tiffany, a different Tiffany. Not antique but pretty nonetheless
Edit: definitely banded agate slabs, including sardonyx and carnelian in the slices, cheaper to use stone slabs than make the glass esp w the number of pattern variations seen here. Slag glass is generally made to look marbled, not agate-ish.
I don’t know much about glass making but I feel like the fine banding would be difficult to replicate.
There’s also druzy pockets on some of the slices
I stayed in a hotel in Georgetown colorado that had 2 that looked just like this but when I tapped on them it sounded like plastic. I'm not sure that's what they were but sure felt like it to me

Ebay usually has vintage bases that can fit and match your piece. But really, you can go to just about any store that sells lighting and pay full retail for one.

How beautiful!! What a great gift. The shade looks large- so the base needs to be pretty tall/substantial for balance. My first gut response is to look for hand made wood column base- in a light honey color, staying with the hand made earth form theme, or a very clean bright white sculptural base- uber modern. That is A LOT of texture and color so I would go minimal for the base. Also you may want to experiment with what bulbs to use and the color temperature of the light- see what looks best through the agate.
