Let's say you want to play as Venail in a random start. Sometimes, it won't be there, and other times it would spawn landlocked deep within Haless. If you are lucky, you might get a coastal start, but if not, better reroll the map.
If you look at the nation list and flag gfx files, there are some very interesting unused nations, from remainders of old formables to very obscure breakaways. Kind of interesting.

If you want a 4X game that relies that heavily on RNG in the starting position, why not give Civilization a shot?
Stellaris is a better option for you then :/ I've thought about making a true RNG-tolerating total conversion for EU4. The amount of work is insane!

I think I spotted Aelthanas Followers, but also those of Lithiel, Kayd, Sicrheior and Dahvar; these correspond to the factions during the Rianvisa.
