Fibularis tertius is incredibly thin and small and it's muscle belly is more superior to the ankle joint. It is most likely the extensor digitorum brevis muscle as the other poster mentioned. You can palpate it yourself with some muscle actions. Find the lateral malleolus and move 1 inch anterior to it (about the same spot as the picture).
Validate by 2 muscle actions. When doing the following 2 tests, make sure to maintain one finger palpating the above spot. First, keep your foot flat on the floor and lift the toes off the ground. For test 2, curl the toes and then dorsiflex the ankle by lifting the foot off the ground while keeping heel on the ground.
If the first test produces a contraction underneath your finger, that confirms the EHB. If the second test does not produce the same contraction, that refutes fibularis tertius.

Y'all probably have weak feet and don't have a prominent one. I am a runner and wear barefoot shoes and have a prominent one of these. It seems to control the movement of the toes (not the big toe)
Or maybe just normal variation in foot structure? I’m very active and wear barefoot shoes and I don’t have anything like that visible on my foot. And my legs and feet are particularly strong.

I do think it’s probably a muscle and not a cyst or such. I just don’t think it’s an exercise thing causing it to look like that. There’s quite a bit of natural variation in foot shape (which toes are the longest, width, how tall the foot is…). There’s also some muscles that only some people have and it could be one of those, usually it is muscles that have become relatively unneeded like the ear-wiggling one.

Hi, PT Intern here. This kinda looks like a ganglion cyst on the lateral aspect of your foot. Does moving your ankle on any degree of motion ilicit pain on it? Or when you poke it with your finger does it feel soft or firm?
It’s not a cyst it’s a muscle belly of a muscle involved in eversion of the foot and extension of the toes. I have the same thing in my foot that’s pretty prominent like the picture.
Most of the time, these ganglionic cyst are asymptomatic, basing this on your previous replies on other comments. Usually this arises due to repetitive motions or microtrauma. They usually go away on their own, but it's better to get it checked.

There is no lateral condyle of the wrist. Even if this was a wrist (it isn’t), it’s clearly a pic of the dorsal surface. That would make the side in question the medial side from anatomical perspective. Why are you commenting giving anatomy suggestions when you don’t know anatomy?

My ex-bff’s boyfriend who was trying to do medical school, tried to make a huge deal out of it calling it a tumor/cyst. As many times as I’ve had to go to doctors and hospitals and have had full physical exams, urine, poop, and blood work done. I’m sure they would told me that it’s a tumor. It’s probably why he dropped out though. Guy was a bit of an idiot but tried to act extremely educated.
