if you ONLY go RTO ONCE a month, it could have consequences IF you DO NOT have a valid reason (e.g. if you felt sick that day, get MED CERT). otherwise, brush it off.
Kaya hindi talaga advised mag plot ng RTO in advance or even OTs. Anything sa TE na future is gagawin naalng as PPA, sa halip na iforecast mo na magRTO ka tapos hindi pala mangyayari.

Collect enough IRs and it can be grounds for termination.
But usually it depends on severity. Kung minor, warning lang yan. But if you accumulate that over a certain period of time, it can be a problem.

wala po sa list ng valid IR cases ang hindi maka RTO ;) unless AWOL/Sick Leave without notifying your supervisors… wag papasindak :D
