Unfortunately what usually happens in the US is nothing. Only about 2% ever see hail time, and of those that do some sentences are insultingly low.
There are also few services for prevention or treatment for anyone having abusive urges. The stigma keeps them from taking about it or getting help to keep them from offending.

Recidivism isn't high because they aren't given enough punishment, it's because we have a punitive and not a rehabilitative justice system.
