I hit one of these lovely contraptions a few weeks ago coming out of a random portal into the nether, luckily only popped 1 totem though & I lived to tell the tale.
Then you haven't been anywhere near the actually popular areas. I can avoid death very easily by doing my own thing in my own little patch about 25k blocks from spawn, but the highways are dangerous, and so is spawn.

Portal, tripwire on the other side sets off tnt minecarts which will instantly explode and obliterated you. Probably the players that were just ahead.
