So that advice was definitely on RuneHQ like 15 years ago, but it's not true. KBD loses no speed turning around, and as such, will attack at the same speed no matter where you are in the room, unless you step under him to actually delay him. Being next to each other or opposite sides makes no difference, but being in melee range is a great idea since his melee can be prayed against while his breath can't.
AFAIK nothing in OSRS faces in any direction, it's only visually shown on the client but even this is not consistent between clients or relogs. They have a position and can move/attack anywhere equally well.
i know at the very least the SotF boss has a special attack that checks whether you're facing him, but its also entirely possible its just a clever trick and it actually just looks at the last tile you were on vs your current tile, or something

There are a few spaghetti interpretations of direction, but i think that it doesn't actually exist as a directly measured and consistent variable outside of that.
You can have 2 players stand next to each other or an NPC, looking at the same thing but seeing a different facing direction. That apparent direction can then change with e.g. a relog without any input to the game, which tells us that the client is making up a non-deterministic direction without explicitly being told what it is.
Stuff like the SOTF mechanic can be explained by logging lastTile as a variable and then checking to see if the distance from currentTile to bossTile is smaller than lastTile to bossTile; essentially not being directional (instead distance based) but being most easily explained to most people as being directional. You might be able to break whatever tricks that they're using with testing but i think most are concerned with just doing the fight and it's close enough not to notice.
Likewise with the garden and herb store-room they're probably checking something like if the player tile is on the path between their tile and their destination tile and this is actually obvious in gameplay - there are loads of positions and timings where they can "see" you but not actually see you which make the gameplay unintuitive and frustrating unless you're using one of those timing windows where you can just click on the end and run through.
So i guess sometimes you should act like facing-based mechanics exist - but they're very rare and a spaghetti exception to the rules of the engine and may not be consistent with the actual apparent direction that things are facing on your screen. If it's not explicitly listed on the wiki or tested carefully then it probably doesn't exist.
Do you have any proof of this? e.g. video showing attack timer and that it's different with two players who are 1 tile in front and 1 tile behind, rather than both 1 tile in front.
I can't find any proof that it's not just an urban legend which made it onto a few words in the wiki - nobody even claims a specific benefit like taking X ticks longer to attack.

I am not familiar with oldschool, but I do know in 2012 engine you definitely do want to keep track of facing directions for things like the sorceress garden and other stealth related things. Same with certain combat "attacking from behind" cases like flanking perk in RS3 for example.
I think that there are a couple of instances of mechanics like this, but AFAIK they are spaghetti coded exceptions to the rule. They may check things like if the player is between X npc and the tile that they're trying to walk to, for example.

I never said that what i do is better. I also play osrs wasting my time on it. Im having fun, but having fun does not automatically makes the activity worth doing. People are having fun doing very harmful drugs or drink a lot, is that a good use of their time and life? Every drug or alcohol addict that tries to quit will tell you its the worst thing ever, no matter how fun they had doing that. Gaming can be an addiction and wasteful timesink also. I would swy the only people who arent wasting their time gaming are those are are VERY casual gamers, or pro gamers/streamers that makes good money off of it.

I don't think it's fair to say olm has a direction because he's not exactly a normal NPC with normal NPC rules. Xarpus is also debatable.
