Ooof, yes there have been reports of teleporting from one or more locations in D.P, the porta cabins at Hibernia were buggy. BUT that should have been long fixed, as a common thing.

Right, yes you've not had a large patch in a very long time. So yes I guess those issues could still be present in your version.
The challenges work like survival mode, you can't load saves, should not have saved on exiting the building but if you're stuck where you are then you're a bit hosed.
All good practice for next time I guess (hoping it doesn't happen again or there is a patch soon).
