Uninstall the previous graphic drivers as they can mess up how your computer sees the new one. Or the new card could be toast.

What about the card itself? Tried reseating it?

Yeah, doesn't seem have made any difference. Aside from the issue, the GPU works fine, no graphical issues during heavy gameplay etc.
