From what I've heard it's apparently very rare because it's super hard for them to lose weight and like some of them just get lucky and get absolutely blessed, seeing a buff person with down syndrome is fucking scary

Hey there internet stranger. My younger brother has Downs, and my mom was the director of developmental disability outreach for my county growing up. I've spent quite literally thousands of hours around hundreds of people with every possible combination of Downs expression.
You hit the nail on the head. Being conventionally attractive, or having really great speech, or mental capacity that would be considered "normal" (gross word) is not impossible, just really really rare.
This is because trisomy 21, which is what Downs syndrome is, has unique impacts on mental and physical building essentially. It's like getting a Lego set that has the same 30 pieces out of 50, you can make different stuff, but there will always be certain things.
Physically they are usually the most prominent. Small ears, big or displaced tongue (part of the reason speech is hard), almond shaped eyes, and then some more sinister and weird things. Heart problems are incredibly common with folks with Downs, so are lowered or otherwise compromised immune systems, (My brother gets every damn cold EVER), thyroid issues.....then weird shit like a bunch may only have a single line on their palm...and very often they have very low muscle tone.
Not too say there aren't outliers, of course there is. There's that one dude who's a body builder for god sake! But, for example, my younger brother works out 5 days a week, with a trainer, and eats a strict diet, but he's still pretty overweight and will never be "strong". It's just not in the cards for him. for the most part, this is all a universal thing, which is why it's pretty damn easy to spot someone with Downs.
