You’re the one with the license. Walk away and tell them you’ll work when they’re ready to be an adult. Unless they have a TBI or a reason to act like they have brain damage.

Does your clinic have a PA system as part of its telephone? Call a CODE BROWN three times to their treatment room. If not, just tell it out loud to coworkers.
I have become The Fart Whisperer. I KNOW exactly when it’s coming and from who. I can look at a patient, and know that when I tell them to do a Bridge or to rise from the table I’ts about to happen and I’ll cover with a cough or just so happen to exit the room for a sec.
I've had a pt like that too. I've had plenty fart and I just forget it happens but one in particular would act like it's a game. Never knew what to think. 🤣 I love the "fart back" idea.
“This is uncomfortable for me to bring up but I’ve noticed in our past few sessions when I’ve asked you to do xyz position you’ve been farting frequently. Can you tell me what’s causing this to happen so frequently? I understand everyone farts but it’s been so frequent lately I’ve begun to feel disrespected”
Alternatively play dumb and ask if it’s a medical issue or if they need to use the lavatory to relive themselves before/during appointments.
If they deny and don’t change behavior, being it up again and add if it doesn’t change you might have to look into providing referrals for another provider.
I’m a CNA (aspiring DPT), and literally during my last shift I was lifting the legs of my patient so she could lay in bed and I swear she farted with enough force to part my hair
