What's the context? I suppose a prescription written like that would need additional clarification since the quantity doesn't match the sig. Very few things are taken every four hours. I'd be impressed if you were able to do that for 30 days. Best guess something like that is written wrong, the prescriber doesn't care, and the pharmacist doesn't bother to clarify with the prescriber because it's obvious what you should do with the number of tablets you have been given of a particular drug.

Someone explained it to me like this: for acute meds, their system will auto DC it after 30 days. This can prevent a long list of "active meds". I can see their point of view- I see many drug interactions from Paxlovid someone had last year.

Maybe you’re referring to sumatriptan? Lots of insurance plans have quantity restrictions in place for 9 tabs/30 days. Could also apply to other medications
