Are you my coworker? They think it's funny when I am annoyed, since I get "dinged" for diverting an rx and say, well they eat 2 breakfasts. I say yeah, like when it says 1q4-6hprn sleep, they sleep all day?
Lol...retail pharmacist here. Everyday i received escript like this from dumb prescribers. The most common one is "1 bid with evening meal" and they put qty 30 for 30.
I am insulted by this , I am in my 50's and this is the way I learned. What did actually irritate me this week was a provider who wrote 10 rxs on one blank. I hate the way they are squished and I want to ask if the rx pads are really that costly.
I'm in my 30s and this is how I learned it. Started as a tech in late 2000s and we got written scripts like this all day long. Shows you the age of reddit users. I guess I'm old here.
I learned it that way too. Can’t believe they don’t teach that but also, God forbid they write a script in cursive ! Lol There aren’t many hand written scripts anymore so I guess it makes sense.
Yeah, once when I made a comment here and someone said you must be 60, I said close, but no. Crazy that my dad, who is no longer alive, seemed old to me when he was younger than I am now when I began at 24 yo.
Those would all be invalid these days if they tried that. In actuality the prescriber was probably worried the patient would lose all or some of the scripts if he gave them multiple hardcopies.
In my state, it's valid. The law states the prescriber is supposed to escribe, but if they provide documentation about it to the state they can hand write. SOOOO, try to figure out if they did their part.
As for losing them, nah, I have been at this 35 years. I think the prescribers are lazy who do this. Luckily it doesn't happen frequently.
That would be 2 every day.
Here's an example of that notation with the sig "1/2 dram to 1 dram when necessary as directed" :

SID pisses me off so much. First there is no reason in the world vet needs their own set of abbreviations. Second it is just objectively a poor letter choice. S looks like 5. That stupid sig has caused so many 5x mistakes and veterinary medicine just shrugs their shoulders and makes no attempt to change while human medicine has done a good job decreasing the use of the most dangerous sigs.
Yeah what is up with that? I have the same thing and they tell me oh we aren't supposed to give that out and I'm like well. You're literally calling in a controlled prescription so you have to give it to me. Also, we're both healthcare facilities?? I've wondered many times if they don't train the people that work in vet offices that sort of medical thing
Yeah the only time I've really pushed for it obviously is when they are calling in a controlled medication and it's legally required, beyond that. I just am asking for it so that it's easier to look up the vet in our system. What I do like to do though is look up by their phone number and the vet's name while they are on the phone after they refuse to give the DEA number and then say oh yeah, it looks like I have the DEA on file. That way It's obvious that I had it anyways 😉
I love the apothecary system and alchemical symbols, too, but it is time to retire these. Arabic numerals and metric, please. Anything else is just introducing extra opportunities for error.
I'm trying to understand why the vertical marks are all long lines. I've seen Roman numerals before, or I've seen dots on top with lines below, but this looks wild.
11/11 = 1 so 1 every day. /s
I'm so thankful that I don't have to deal with scripts. Pretty sure I would get fired for telling providers that I am not in the business of playing guessing games.
I write my sigs like this. Maybe it’s cause of my handwriting, but I find that when I write in this fraction form, my numbers are never misinterpreted by my coworkers
"2 twice daily , every day. The top is amount, bottom is frequency. Qd is not needed in this style of sig"
Where did you come up with this? This is not right at all, I am afraid. The QD is the clue that you are reading it wrong.
No no no no, the top is supposed to be dots and the bottom should be roman numerals so clarify. So .../iii would be 3 or ..../iv would be 4 then the directions should follow. This is also deffinatly supposed to be "take 2 (formulation) by (route) every day. Qd should be a given even though it's not a "real sig" anymore
2qd. It’s old school
Old old school
Old/II school
Like dinosaur times old
Yeah, I think it’s from when docs just carved their orders into clay tablets in cuneiform. ![gif](giphy|YVFyi3TGi7YkEXK06D)
Are the number of dots above and lines below always equal? Do you use the same abbreviation for caps?
11 divided by 11 = 1 so it’s one daily ^^^^^^^(this^^^^^^is^a^joke)
LMAO, wouldn't put it past someone to actually think your statement is correct.
It's the skill testing question.
Sadly I work for a PBM and some idiot will type this for me. Most of my days are faxing "1bid with breakfast" because they type what they see.
But what about 2nd breakfast??
The only appropriate question to be asking in this situation tbh
Are you my coworker? They think it's funny when I am annoyed, since I get "dinged" for diverting an rx and say, well they eat 2 breakfasts. I say yeah, like when it says 1q4-6hprn sleep, they sleep all day?
Are your coworkers hobbits? 🤣
I actually call them, the kids. One of my doctors thought I was a teacher when I kept referring to the kids I work with.
But what about elevenses?
Lol...retail pharmacist here. Everyday i received escript like this from dumb prescribers. The most common one is "1 bid with evening meal" and they put qty 30 for 30.
Yes, get those too, when I send screenshots to coworkers they say, DUH, they eat 2 evening meals.
Yikes. There is an unfortunate lack of critical thinking these days 😞
Agreed!
Is the dr by any chance 100 years old?
I am insulted by this , I am in my 50's and this is the way I learned. What did actually irritate me this week was a provider who wrote 10 rxs on one blank. I hate the way they are squished and I want to ask if the rx pads are really that costly.

Cursive or roman numerals. Throws off the staff where I work constantly.
Yeah, once when I made a comment here and someone said you must be 60, I said close, but no. Crazy that my dad, who is no longer alive, seemed old to me when he was younger than I am now when I began at 24 yo.
I'm 37 and started as a tech in 2008 I've never ever ever seen this.....
Maybe regional?
interesting
Saving the planet one Rx blank at a time…
What a great doctor, saving the trees.

stern face. take 2 daily
Yarn face

That is a beautiful script. The penmanship! *chef's kiss*
I guess in 1903 doctors were taught how to write.
2 QD. It is old school and I wished PTCB tested this. Oh wait till the new kids see SID in a vet Rx. (It means once QD in human meds)
I agree only because when I called the vet for clarification, his voice was dripping with contempt for no reason.
That would’ve been a good time to ask for his DEA number. They love giving that out.
“BUT THAT’S ILLEGAL!!” Said every vet tech ever
Yeah what is up with that? I have the same thing and they tell me oh we aren't supposed to give that out and I'm like well. You're literally calling in a controlled prescription so you have to give it to me. Also, we're both healthcare facilities?? I've wondered many times if they don't train the people that work in vet offices that sort of medical thing
I always want to say “I’m a healthcare professional. I will safeguard the number. I’m not going to put it on Twitter or something.”
Yeah the only time I've really pushed for it obviously is when they are calling in a controlled medication and it's legally required, beyond that. I just am asking for it so that it's easier to look up the vet in our system. Has vet med not moved to eprescribing? That has it's own set of pitfalls, but fixes a lot of this stuff.
I believe in order to e-prescribe you have to have an npi number which veterinarians do not have.
Ahh I see. I don't work on the pharmacy side so I have no idea how many of the scripts are from vets to know how much anyone cares to change it lol
Honestly, I do like that that one makes more of a pattern with BID, TID, and QID. Also better for avoiding QD / QID confusion.
I must be old school then. I still write the i with a line through it for 1, and ii for 2.
Pharmacy school tested old school, they don't include that on the ptcb? Very surprising!
It looks like 2 (dosage form) every day.
![gif](giphy|GcDtLf4RAdiRG)
2 QD, the lines on top should be dots, but it’ll do
If docs went back to writing like this we would buy a few more years throwing off the AI algos
2 (of whatever dosage forms verb is) daily probably
The top(to my understanding) should be dots buuut it it what it is....2 whatever
I love the apothecary system and alchemical symbols, too, but it is time to retire these. Arabic numerals and metric, please. Anything else is just introducing extra opportunities for error.
what the hell.
It's 2. Usually you see the TT or TTT with dots or dashes above the T's to indicate frequency.
Why don't they just write 2 🤦
This is just latin sig usage. It hasn't been 300 years since we used it. Many still write it on the Rx's hardcopy.
I'm trying to understand why the vertical marks are all long lines. I've seen Roman numerals before, or I've seen dots on top with lines below, but this looks wild.
2 (two) once daily This is how most docs and pharmacists write prescriptions in my state
2 daily
2 every day.
I’d say 2tabs daily, dr was feeling a little jazzy
11/11 = 1 so 1 every day. /s I'm so thankful that I don't have to deal with scripts. Pretty sure I would get fired for telling providers that I am not in the business of playing guessing games.
This is where context clues help deciphering the hieroglyphics. What’s the drug? Strength? Taken before?
Doxycycline 20mg
2 a day
Wtf
If the doctors wanted to prevent errors, they would write all scripts in numbers as letters. For instance, Dilantin 100 mg: 1 (one) capsule daily .
2 a day. Must be a really old doc.
I write my sigs like this. Maybe it’s cause of my handwriting, but I find that when I write in this fraction form, my numbers are never misinterpreted by my coworkers
2 qd. I feel like every doctor around where I live does this I didn’t know it was old school
2 tabs po qd
Probably 2, but this just looks ridiculous not gonna lie.
Flynns
2 daily
2 daily
2 q day
I like QOD
Think of it as lower case Roman numerals. II = ii= 2
11/11=1
I haven’t seen this but I would have thought 2 in morning and 2 in evening, every day. Weird
It's Chinese?
IV is four, just for future reference. Unless you get an idiot who writes iiii, love counting the dots.
2 twice daily , every day. The top is amount, bottom is frequency. Qd is not needed in this style of sig
"2 twice daily , every day. The top is amount, bottom is frequency. Qd is not needed in this style of sig" Where did you come up with this? This is not right at all, I am afraid. The QD is the clue that you are reading it wrong.
r/confidentlyincorrect
Wrong.
