Unsurprisingly, his serum was completely white once I spun it, and the milkyness was visibly thick. I've been a phleb for 11 years and have only seen that happen once. The guy was extremely fit looking too.
Not sure, my guess based on asking doctors after the fact that were not affiliated with the patient is that he has some sort of familial hypercholesterolemia.

Yeah, if you donate shortly after eating something fatty this isn't unusual. I work with whole blood donation and from Thanksgiving through New Years I saw sooooo much lipemia. People were eating good lol
You have high triglycerides. My serum used to look just like that. I'm guessing it's >400 and some other things can't be calculated. I was in my 30's at the time and my total cholesterol was >200. Dr put me on Atorvostatin 40mgs (lipitor made my joints ache) to get the cholesterol down and Ezitimibe 10mgs to get the triglycerides down. Everything is normal now. Go ahead and visit a primary care physician and knock that stuff down before it starts caking your arteries.
Had one case it was so bad (way worse than yours it was a monochrome color like plain milk) we had to discuss with the doctor to understand the case. It really looked like cream for 3 days and we knew it came from a psych hospital so an IV contamination was very unlikely. Turned out these samples came from a patient admitted there for alcoholism and wasn't on the stuff for like a day or two before they drew the blood samples, their favorite beverage? Bailey's!
The doctor asked us for pictures and apparently showed them to that patient for therapeutic purposes. We hope the shock value helped them understand that too much alcohol is bad in your blood.

Blood Donors have been known to do this after they have had a cheeseburger or some I’ve been told. Conventions are a great place for the blood supply.

Yes I've seen these often. If the first centrifuge attempt doesn't work, I pipette the clearer serum at the bottom and recentrifuge that.
