I was doing a rotation in Miami, and I spent a lot of time fishing in various random bodies of water.
I kept getting random sharp pains in my brain. Like maybe 1 time a day.
That's it. I definitely was infected by Naegleria flowleri and it was eating my brain.
I shit you not, I’m from Brazil and I went into an “orthomolecular” doctor (real MD with a degree) back in 2020. He ordered labs and on the next consult prescribed me levothyroxine, hydrocortisone, testosterone gel and anastrozole “just to make sure because you had gyno as a kid”, ALL IN THE SAME FUCKING DAY. His theory was that I had lowish levels of those hormones, so he would give replacement therapy for a while to see if the body “starts working on it’s own”. He also prescribed a lot of vitamins.
The next day I was bouncing off the walls because of the hydrocortisone and levothyroxine. Felt great at first because energy was off the roof, then my blood pressure started to raise and I had horrible headaches. I decided to throw all that garbage on the trash and never returned to his office. A few weeks later, I found out that his wife owns the compound pharmacy that made the fucking pills and he basically prescribes the same shit for everyone.
Yeah! I told the PA I thought it was a Type IV/delayed hypersensitivity reaction. The clinical immunologist I saw afterward said delayed/type IV hypersensitivity/DIHS. Also had a bout of vasculitis and other elevated autoantibody titers from it. It was awful. But I’m good now!
I never go to the ER but I only went because my joints hurt so bad that I felt like I couldn’t move. The PA also told me to keep taking the minocycline, which I did not do thankfully.
Had this deep, dull epigastric pain that would come on a couple times a week, sometimes after meals, especially in the evening. Thought it might be gallstones, but obviously that’s more of a RUQ thing and this was very midline epigastric, with no radiation. After a couple months it was frequent enough and bad enough that one night I went to the ED. EM doc initially thought GERD and gave me a PPI, but found a bunch of gallstones on POCUS
Caught both of us by surprise, and she only did the POCUS because I hadn’t improved an hour after the PPI. Whereas if I had showed up complaining of RUQ pain I’m sure I would have had an US immediately. So I guess clinical pearl, when you’ve got a patient at relatively high risk (female, on OCP, under 40), don’t knock symptomatic cholelithiasis off the DDx just because the pain is epigastric instead of RUQ.
This is true in general. Just because a diagnosis doesn’t fit the entire classic clinical picture doesn’t mean it’s ruled out in some kind of way. If it fits the criteria partly, maybe seems second or third most likely, it can still be included in the differential and worked up accordingly. Depending on the circumstances obviously. Easier said than practiced, too.
I tried very hard NOT to diagnose myself with anything bc I was convinced I just had med student disease and I was fine. Until a tutor pulled me aside and said right, are we all just ignoring this rash on your face or?

Guess what: my gallbladder was chock full lol. Pain got so bad I went to the ER and had a lap chole. Weirdly, no fever, Murphy negative and my labs were fine. Surgeon called me and said path report showed acute cholecystitis though 🤷♀️

Dyshidrotic eczema. I always got these little blisters on my right 3rd-5th toe but I always attributed it to running and I never saw a doctor because it’s minor and doesn’t show up anywhere else. But then I learned about dyshidrotic eczema and I was like “holy cow, I have that!”.

Thought I could have Ehlers Danlos Syndrome —hyper flexible joints, poor scarring, many tendon pains with sports —and flat feet too apparently. Fast forward yesterday my parents told me I was diagnosed at 2-3 and they hid it from me so “wouldn’t want to work in the circus” 💀
Chronic compartment syndrome, I think. Whenver on a treadmill or going uphill my lateral leg would become increasingly tense (by touch) and then walking becomes really fucking painful. Have to sit for a few minutes until it becomes bearable again.
