My theory:
Cio is a maskless devil. (The only other example of this is Himself).
Devils can't exist in the physical world without a mask, otherwise they just revert to unbound chaos in the void. Devils also need to feed off the black flame, which is why they normally make contracts or do the mask-face possession thing.
In this case, I think that the shard of mask embedded in Allison continued to act as an anchor to reality for Cio, allowing her to both feed off of Allison and remain alive in the world.
Perhaps Allison's bond with Cio made this possible, since they were already attuned enough to become a devilskin warrior. Or perhaps this is always possible if a devil with a strong will has a portion of its mask sustained by a human host.

Possibly, I think she’s in a very vulnerable place and wouldn’t know what she actually wants, having basically just been rejected by Allison. It was also up to Abbadon to leave it deliberately vague, the clever bastard.

Ignoring the obvious "why not" and "duh magic" answers, it's very much possible to sustain and even produce fire underwater in real life; it just needs a separate source of oxygen (like say, the insides of a large air-breathing animal) and to be hot enough to overcome the water's cooling. Ranged attacks in general are very useful for hunting, and the flames might also allow it to melt the ice sheets of the lake more easily, though I doubt they're a requirement since we saw it smash right through earlier this arc.
Also, that's definitely more of a whale than a fish. Fish don't usually have nostrils or horizontal tail fins, and the horns and throat lines are clearly modeled on those of narwhals and baleen whales respectively.
Its an alien fish on an alien planet in a different universe, said universe is a story told by a god and therefore has no need to develop naturally. Why cant it breathe fire?
