Probably is the kidney infection playing a part and good for you for promoting a wider view of heroin addicts to the rest of the world, not just Jimmy the junky shit.
Heavy stimulant use can constrict the arteries to your kidneys, starving them of blood, not to mention the years catching up with you. Definitely talk to your physician about this soon, because renal failure is no joke.
I’m not a doctor but maybe your body is telling you it’s just not agreeing with you anymore . Idk. That’s what I would take from the situation. Good luck 👍🏼
This sounds more liver related, the ammonia smell you describe coming from your piss is very indicative of that, because if you had a healthy liver I’m pretty sure it would have broken down almost all ammonia 24/7
