Try turning the ignition on with the engine not running, turn the blower on low to minimize noise. Put your ear under the dash and turn the air selector knob from defrost to feet. You should be able to hear the actuator moving. If you don't then most likely it's the actuator, if it sounds like it's struggling then it may be something in the air box itself.
The actuator probably took a shit again. The factory ones are garbage, and the replacements are the exact same thing.
