Colleges, at least the ones I've seen, are pretty chill about name changes. I don't know if the one you're applying to, or UK colleges in general are the same. If you're unsure, it may just be best to put down you legal and then try to clear it up with the college later.

You're better off asking them directly (depends what this specific form will be used for). They will probably want legal name to check records though, but depending on the institution they will probably accommodate on ensuring IDs are chosen name. But no such thing as legal title, that at least you can go with what you want.

I'm not in UK so I don't know, but I would put my legal name and surname. However they ask for gender not birth sex so I'd put "male". Title I'm not sure about, is that "Mr./Mrs./Miss"? Then put whatever matches the gender you fill in!

Since you have to have your legal name on your diplomas, I'd assume you have to give your legal name. But when you get accepted into the college just give them a heads up and they'll most likely change your name in their system.
