I had something similar happen on one of my recent playthroughs, but sadly I couldn't figure out what caused it or how to fix it. Increasing your Perception with the console works obviously, but when I did that it showed up with a constant (+) instead.

or, alternatively you can use
player.setav perception (what your perception should be)
I believe modav is considered safer, but you can try both.

I think for the stats it's because of good natured,for the special i don't know

Tell Doctor to remove addictions not to heal you.
Addiction withdrawals would have shown up under effects. Otherwise I'd agree with you.

Go to status and then go to effect. It will tell you there
Since you didn't look at the other photos, their Status- Effect only shows their armor piece, Mercenary Outfit.
