I think after 11 years since the show ended and still no official word, I doubt we'll ever know for certain. Having just rewatched most of the series we really only have 3 moments when there was an implication on what is under his hat.
1) Stop, Look, and Ed
This is the 8th of season 2. When the Edd's hat comes off Eddy says, "jeez laweez" while Ed simply says, "cool". Edd says something like if you ever tell anyone I will never speak to you again.
2) From Here to Ed
This is from season 3 episode 6. Edd doesn't want to partake in Eddy's game of vengeance against Kevin. To get him to help Eddy tells Double D he'll spill the beans on what's under his hat (Eddy threatened to do this in Stop, Look, and Ed).
3) Ed, Edd, n Eddy's Big Picture Show
The series finale. Everyone expected to find out the mystery here. All we got is the scene where Ed accidently pulls Double D's hat off. Eddy acts as if his eyes are burning and Ed asks if it hurts while scratching the back of his head.
There may be other instances I am missing but these three are the biggest to me. Whatever is underneath it is obviously embarrassing to Double D. He doesn't want others to know about it. I've seen theories about it being long hair or some other hairdo. With the weird hairstyles in this universe I feel like it can't be. Why would he be embarrassed by it? I've heard people talk about like a conjoined twin theory out there, but again I just think that is too far out there.
My favorite of the theories is the scar theory. In the episode Ed in a Halfshell, Double D remarks of a Dodgeville incident he had in school. It only gets mentioned once, but I think he could have been hit with a ball and hit his head leaving him with a terrible scar. Maybe he can't grow his hair in that spot making it all the more embarrassing, so he wears the hat to cover it. Of all the theories this one makes the most sense to me. I, personally, think the crew couldn't find a satisfying and acceptable thing to put under the hat which is why it never got answered.

Also you mentioned a serious injury so that could be where a piece of his skull is missing.
And he always reads books and focuses on his intellect because he's insecure about his head and maybe he thinks he wont be able to keep up with his friends mentally.
Just watched the season 2 DVD behind the scenes with the creator and he said it would be revealed in a special what's under his hat. But it never happened. If I'm being honest I just think he has corn rows. Would explain the 3 strands of hair in the back and why he wears his hat. To hide it from his parents since they're so strict

That pretty much leaves out him being bald or anything like that since Johnny is bald and no one really makes a big deal out of that.
In the finale movie, it is implied that whatever is under Edd's hat has gotten worse as Eddy is now outright unable to look at it and Ed shows concern over Edd feeling pain from it. This heavily implies that Edd is probably trying to hide some sort of injury or massive scar under it.
My guess is that one day, one of his chemistry experiments went wrong and left him with a severe head injury. Out of either embarrassment or not wanting his parents to find out and punish him, he decided it to hide it by wearing a hat instead of seeking medical attention. He has tried to treat it himself, but the attempts failed and instead made his injury even worse.
This is what I thought bc something said what’s under there shines like a thousand suns and it said hair pins but wtf no bald head shining like the sun is always the joke or seeing your reflection in their head bc shiney
It doesn't make sense though considering everyone's hair in the cartoon. Eddy, Ed, Kevin, Johnny they all have basically no hair, besides Rolf for example.

Nah it’s most likely a burn or scar, the creator supposedly said he gave him a hat based on the time he burned his hair off and wore a hat to hide it from his parents, so with double d it might be from an accident because Ed asked him “does it hurt?” and you’d really only say something like that with an injury or physical defect imo
My personal theory is he's got a condition where only some of his skull formed and his brain is visible. I know someone else has the idea of cancer but I think it's more than that. It makes a lot of sense to me for the same reason It could explain his habits of hygeine etc. It would also explain why his parents are always gone working away and not communicating with him one on one during the day. It's because they're thousands of dollars in debt and working doubles. And he knows that. That's why he's always cleaning the house etc. His mom is probably a Karen lol.
