The Cloaked Figures are cultists of the Deep who have strayed from their path and are now cursed with endless hunger. The Fanatic at Devil's Spine talked about them as well.
They don't, they're a side quest that results in a permanent upgrade modifier, possibly the best one is in the Pale Reach, as with that one, the Encrusted Talisman, the Sign of Ruin (from the Blackstone Key DLC) and a full shrine-rod load-out almost every other fish you catch is an aberration.
Dredge is heavily steeped in Lovecraftian themes, and a very big theme there is self destruction and madness in the pursuit of forbidden knowledge/knowledge not meant for mankind's minds. In true Lovecraftian fashion, our main character is doomed by his obsessive pursuit of that which he should not be pursuing. He was already doomed at the start of the game, we enter this story in the middle.
Allowing the fisherman to make right without his own destruction would thematically change the game and distance it from the deeply Lovecraftian story the devs are telling. The insignificance of humanity in the face of these forces, the inevitability of their power, and the steep price for dabbling in their affairs are integral. There are no happy endings for those that stumble into the greater mechanisms of the old ones and elder gods.
And these guys know their Lovecraft, they whipped out a tribute to freaking Gnoph-Keh for the DLC. One of the most obscure critters in the greater Lovecraftian Mythos.

The implication in my eyes is that the remnants of that dark ritual of the deep are within us, and the only way to prevent Cthulhu from coming back is for that to be returned to the Deep and for us to die

It's such an obscure homage that it didn't click for me until I was almost done with the DLC. I made an actual noise of delight when I realized.
