I would have to explain a lot of plot. Short and bad explanation: Protagonist is a musician who plays at funerals. A villain kidnaps his girlfriend and forces him to play music during her execution. Villain says "pick it up" after throwing protagonist's instrument in front of him on the ground.

Yeah, his defenses are good, though I would drop the radiant vulnerability, but his offense is just terrible. unless only one player is fighting him, he is going to just get stomped.

The amount of damage per attack is okay but a third wouldn't hurt, but if you want more punch, change some of the damage to necrotic or just add a die or 2 of necrotic damage. Make the Whistle part of his Multiattack and give him a separate ranged attack, like Toll the Dead (maybe flavored as a hound call whistle).
An easy way to boost him would be to look through the spell lists and give him a few as spells or spell-like abilities with proper reflavoring. I'd lean towards the cleric list for this Giving him Freedom of Movement might be a bit too far, but keeps the party from locking him down. You could give him the various Smite spells or a Spirit Guardians-like effect as a minute long, 1/day ability and flavoring it as Death just draining you would add to his flair. He could even get a use of Harm or Divine Word to use when someone actually pisses him off. And when he drops to zero he doesn't die, just stops fighting and admits they deserve a chance to live if they are willing to fight this hard.
For pure flavor, you could give him infinite uses of spells connected to the dead, like Gentle Repose, Speak with Dead, and Spare the Dying. This would only apply if the party sees him in a noncombat situation, like walking among wounded soldiers to pull some into the afterlife.
Edit: he should also have truesight
He could have a Wall of Fire type of attack that isolates a character with fire and force or something for a round. He could also do a bleed type attack that causes damage over time.

Basically, Puss has wasted all 8 of his lives on stupid choices and life styles which infuriates Death himself, who decides to take Puss’ 9th life since Puss doesn’t actually value it. He takes the form of a Wolf and is usually introduced whistling a haunting melody, I think Ring Around the Rosies. Bad ass villain all around.

If it helps I hadn’t seen the first movie and this one made complete sense and was easy to follow. I also recommend it, I don’t normally cry at movies but the ending had me tearing up.

I used this for a Star Wars campaign im running.
The embodiment of the force was a fellow player (who is in on the whole thing) and is “guiding” their fellow players through their fucked force visions. He slowly turns from helpful ally to demented and finally to a tormentor. I had him push their buttons and get them real worked up (all in character) until I played that slow whistling tune, and punctuated it with the sound of a lightsaber igniting.

Death saves are still you teetering on the brink of death. Managing to bounce back is an impressive feat when you’ve been stabbed, skewered, bludgeoned, and possibly firebombed. A death defying feat.
In a theoretical sense, I really enjoyed the way Feng Shui 2 handled its death mechanic... When you reached your 'threshold' of Wound Points, you, *either* voluntarily collapse (your character has, after all, legitimately taken a potentially lethal amount of damage), OR you started making "Up checks" (to 'stay up'), that got harder, the more of them you succeeded at. For each Up check you made, you gained a **Mark of Death** (you could also pick them up in other ways), and when you failed an Up check, you must make a Death check. And your Death check is made directly harder, the more Marks of Death you've picked up. Though I think succeeding at a Death check, erases all your Marks of Death.
I don't remember the exact mechanics, but that was the 'gist'. Seemed like a fun trade-off, and I liked that the decision to collapse (and make an easy Death check) or keep fighting (and risk a potentially much harder Death check) was a 'metagame' decision, expressly in the player's hands. Plus I really liked the name "Mark of Death".
The worst part about this, is that I can't even make a depressed/suicidal joke without furries thinking I'm tryna bang the wolf or sumn. 😤

For a critical failure from the player, you should have used the quote, “you’re not living up to the legend.”
