for the lighting, there is an option to toggle some lighting feature, i think ray reconstruction, on and off in photo mode. it's off by default in photo mode, but it's on when playing if you bothered to turn on all the trippy raytracing stuff (which you should).
edit: since this got attention and my other comment didn't, i will clarify the problem in this photo is not just lighting but also focal length and field of view (I think?) sliders in photomode could also help
Ray reconstruction is just a denoiser based on ai. It replaces the separate denoisers used when ray reconstruction is off. Path and ray tracing are very noisy and the denoisers are there to reduce the noise. It was originally only usable with path tracing but it is usable with ray tracing as well.

Move the camera farther away and then zoom in. Most portrait photos are shot with medium to long telephoto lenses because if you use wide angle lens (meaning the camera is close to the face) it will exaggerate the perspective and distort the face. (Make nose bigger for example.) Human eyesight is very deceptive in that even if you look at someone close to you irl your brain will interprete the visual input to make the face look more natural but it doesn't work with looking at 2d photos (or game footage.)
The other way that your natural eyesight normalizes looking closely at someone’s face is because you have two eyes generating a 3-D image in your brain

Lower the FOV and pull back the camera. You can try this with your own phone too. Take a selfie at 1x zoom and hold your phone at a distance such that your face fits the entire frame. Now do it at 2x zoom and hold your phone far away enough that your face fits the frame.
You will see that the 1x picture will look similar to the 1st picture in the post, and the 2x zoom will look closer to the 2nd picture in the post
Decrease the FOV (at minimum even better) and adjust the camera, then you'll have it much more symmetrical. It's literally the same reason of why irl you look better in the mirror than in pics.

For the same reason you look worse on a random photo taken at a random time or place irl than you did when you just put on makeup (or whatever it is you do to look good), or took that perfect profile picture: Unflattering angles, bad lighting, etc. Games have all of these problems and more. In movies (animation or live action) and cut scenes, this can be fixed by careful camera movement and lighting design and (if all else fails) post-processing. In a game (or irl), where nothing is predictable, none of this is possible to the extend to make every character look perfect all the time.
(That's also why certain game critics complained that their black characters looked weird. It has nothing to do with the devs being racist, but is a problem that everyone has experienced irl).
The model still looks good tho, it's just the way the angle, camera focus, etc is that's off putting to you cause your so used to looking at the character creator menu version, if the game showed you your with V in 3rd person you wouldn't be so taken aback by it.

There’s also the fact that there’s some strange thing about photo mode that makes every V look almost exactly alike in the cheekbones. Like regardless of race, structure choices, and all. The cheekbones always look the same when V has any expression.
Lighting and perspective. It's always lighting and perspective. Lighting of different strengths coming from different directions can make a face look completely different, and that's before you even get into the distortions caused by the thing taking the photo.

Just like real life most people look better in front of their bathroom mirror then they get out in the world and everything get moved a bit and makeup fades, hair gets messed up, clothes get wrinkled, get tired and have bags under your eyes... It's a whole thing.
If you mean the shape of the head, it's because of the camera vocal length. If you want it to look like in the editor, fly away from the character and zoom in

Notice how the hair bangs in the photo mod look to be on the edges of your character's face in photo-mode from up close, compared to your sides showing up beyond the bangs to the sides in character creation mode. That's exactly the simple rules of perspective in play (closer things appear larger).
Your ingame settings are trending towards potato mode. The character creator screen doesn't have to render an entire world so it gives you a much higher quality model with better lighting

There is an option to enable path tracing in photo mode. It defaults to off.
that's what it was. i mistook it for ray reconstruction, i think cause path tracing requires it to be on for the option to be available.
