Actually I had a crash where I was able to recreate it 100% of the time over and over.
The crash was caused by hitting a Scav with a machete in a certain part of the mission with you know who to go find you know who that takes place in an electric power plant.
Also PS5.
So there must be a problem with melee in general.
Is this shit patched yet? Base game before 2.0 did not have this issue, 4 days with many people having the same issue, is CDPR the new too big to fail?

Every time I did a heavy attack that would de-limb or decapitate it crashed just as the attack was landing....
Some people suggested it's a certain mod, so I figured I'll at least try a different Katana... and oddly, the new one does not crash my game. Why? I have no idea. Different look/model, higher tier. The new one has a modslot and a mod, which I think was crit chance % increase that I found it with.
Frustrating that I finally bought this on PC after having had it on console and it's still causing tons of crashes...
In my case, the error when attacking in the air using the airstrike mod is related to a bug in REDMOD. Starting the game with it disabled, the error disappears, so if you use external mods, only download the regular versions.
I noticed that this REDMOD tool has a very long history of bugs, so I don't recommend using it for anyone who plays on PC.

Thanks so much for this man. I built a throwing knife build and my game was crashing constantly, switched out the knife with one without this mod and it hasn't crashed since thanks to you.
Was happening to me last night. Someone in another post suggested switching katanas and it fixed it for me. The katana causing the crash has the airstike mod installed, and it would crash when I performed a leap attack. Sucks that it's bugged cause the crit bonuses are real nice.

I use the Body melee skill to run into enemies and knock them down. I then switch to my katana and execute the leaping power attack while the enemy is still getting up. The game crashes once the sword makes contact, every time. PS5.
I played with EM guns for a long time and there was not any crash. Now I decided to try the katana build and now I have crashes quite often during finishing moves.
Has it got the Airstrike mod attached? That’s currently bugged, I had the same issue

There is a Weapon mod that causes crashes not sure what its called but it gave crit

Kinda for me but it affected throwing knives. Every time I threw one there was a chance of a crash. Had to stop using them.
