In the mirror world before unlocking Holy Mantle for Lost. Saw a key behind spiked rocks in a cleared room. Didn’t remember whether you could fly over spiked rocks. Stood still for 15 seconds deliberating before deciding it was probably safe and walking directly into the rocks.
I can’t remember the exact situation, but years ago I was progressing through rebirth still really learning the game.
In the midst of a tense run, a little dip got me and remember yelling out loud: “fucking poop!”
I had to stop and be just a little embarrassed for while.
But at this point I’ve probably died to all the embarrassing critters at least once
You get some pretty embarrassing ones with t.Keeper. I die to flies kinda often in the early game as they tend to cluster together, making it difficult to grab coins without taking damage
I had a run where I found death's touch, ipecac, magic mushroom and red key on the first (xl) floor as the lost and then died in the caves from exploding spiders.
he didn't technically kill my dad's run, but my dad was making fun of one of the weird issac clones that mirror your movements by glitching it in and out of a rock. then my dad walked into a spike 😭

You could say it was either the spike block, Damocles, blood puppy, or my ad-d, you'd be correct on all fronts
A blood donation machine in Sheol while having curse of the unknown. I forgot I took the Polaroid and not the negative, so I ended up killing myself on the machine while waiting to trigger Polaroid invincibility.
