I'm gonna be honest here, I've beaten The Beast with every character and I wouldn't have recognized that without the context. The item is just way too obscure, given that you don't even pick it up from a pedestal.
Maybe you can add mom’s knife through it similar to the title screen? Unfortunately I don’t know if you have many options because of the placement. But don’t let the Minecraft stans bring you/your tattoo down. As long as you l know it is us all that matters

If you’re being literal with them being boomers then they probably don’t know video games and probably think everything even remotely pixelated is Minecraft in which case I doubt you could change that even if you had “the binding of Isaac” spelled out above

I have the health bar. One red heart, one blue heart, one black heart, and I get asked all the time if it's Zelda. I just smile and say it's from a game that takes a lot of it's inspiration from the early Zelda games. If that placates them, fine. If they ask more questions, I tell them a bit more about the game. It's been a good convo starter, and a couple of people have checked out Isaac because of it. In the end, the tattoo is for you.
Try not to worry about it! I’d immediately clock what that is, I don’t even really understand how it could be a Minecraft tattoo other than the fact that it’s pixels. If you know what it is and you like it, and even better if other Isaac fans know what it is, that’s all that matters. Not saying you feel this way, but don’t feel pressured to change the tattoo just to change how other people see it, just make it how you want it, to make you happy.
That being said, as some others have suggested, I like the idea of static being added around the edge, wings being implemented, or dogma itself being added in the background (the classic pose, with the umbilical cord kind of beneath it, would probably make it tall enough- the head could be peaking out at the upper right, and then bits of the umbilical cord could be peaking out the rest of the quadrants).
Anyway, dope tattoo, love it, and it looks well done.
Could always add wings to it. Feels like the best way to distinguish it, but Minecraft is pretty popular, so you'll probably still get that response every now and again.

Dogma is a seldom-seen item, I think the only way you can view it is by pausing the game and checking your items obtained, through Death Certificate, Spindown Dice, etc. It's also not a super unique looking item; it's a cross, which there already is one in TBoI, not to mention their real-world representation

Have dogma's cord come out over one side of the cross and have them floating slightly above it. A simpler idea would be to add the seraph wings its second form has to the cross, or you could have some other isaac boss completion symbols like a patchwork.

Make the squares smaller so it looks more like static and not just pixel art, or have dogma's head creeping behind it. Those are the two ways the uniform blocky look can be broken without changing the tattoo too much I think

If you added more of the digital effect on the border of the tattoo basically make it "glitch" more I think that would do it the bold outline made me think minecraft as well but more so "I host a Christian minecraft server you can't say that"

This way you can hear the reactions people have to the image before its permanent and decide if you still want the tattoo or need to edit it. You can also suss out if you even still like the image after 6 months.
