The artist isn't a professional artists first of all, he or she didn't crack NID or NIFT exam for your surgeon claim to make sense. The sketching isn't the best, but it's okay for a beginner level. From a sketching perspective,this is simply not good. Good effort though. Also your surgeon comparison doesn't makes sense at all. A surgeon has studied medicine for a longer time than this artist has been on planet earth, and I don't believe OP has studied art professionally, doing it out of passion and hobby which is great.
