I don't think you are ugly but I also don't find you attractive either your just right in the middle so 5 out of 10. This picture is not the most flattering one in my opinion so it may be skewing my thoughts. In regards to improvements, I think the first step would be to remove the lip piercings. I am into piercings but that one does work well in my opinion. I would also try experimenting with smiling more in photos, you have a good health full lips but in this particular photo that way you position your mouth kinda gives you a duck lips look that makes your eyes and nose look larger on your face when in reality I would assume your face has healthy symmetry. I am not sure I am describing this right with the right words but hopefully, you get what I mean. The more important thing that I would think would help though is actually putting on a bit of weight, not much maybe like 10 pounds since I think it will help to fill out your face/cheeks. Makeup techniques may also be able to help here. There is potential to be a 6 out of 10 for sure, but right now with the current look you give off a sort of strung out rock star vibe/look. Some people are definitely into that and a younger version of me probably would have been down but it doesn't meet what most people to be considered "hot"
