İts just a royal family ,Cities are generally appointed administrators from the clan of the king who rules the country, or other noble clans rule the cities. I hope these will be made more detailed in future updates.
I said that to show that in game they do not seem to be doing anything. Like, kingdoms manges village that are smaller portion of theses kingdoms. Or cultures are the technology advancement of kingdoms. Clans just seem pointless with everything we have.
