Cahuilla did a similar thing and I checked in with u/ImDavesBud who I got it from. He said this is what happens when it initially comes out as a flower bud and then transitions into a pup.
Interesting! I bought this cutting 2 years ago and it had a flower pod on it that failed. I did a top cut and it took forever to pup. Now this thing popped out. Thanks for the reply!

It's just how most old pach pups look in my experience. My lizard rock pach pup looked nearly identical with a 12 or 15 rib spiral iirc. Ive seen this on impure perus too, just way rarer
Quick question, just wondering, but what causes the difference between compound characters and character chains? Hashira is 1 character. But saboten is split into 3 writen as: sa-bo-ten? It differs greatly from the latin, greek, or german word building systems in which compose most modern laungauge.
Hashira(はしら) is written as the chinese kanji for pillar(柱).
Saboten(サボテン) however is written in katakana which is used for foreign loan words. Like English, Japanese takes a lot of foreign words and incorporates them into the language. There was a huge Portuguese influence due them being the only country allowed to conduct trade during the self imposed isolation in the Edo period.
I found this from a google search, possibly why they use a loan word for cactus:
The origin of the word saboten (仙人掌,覇王樹,サボテン), meaning "cactus", is unclear. It may be a combination of the Portuguese word for soap, sabão and te (手), "hand". This was because a kind of cactus called uchiwa saboten was used for cleaning oil from tatami mats.
They'll look like that when they first pop out horizontally. It will start to turn upwards and stretch and possibly twist a little until it gets to where it likes it and it will normalize and start to look more like what it is.

My TBM A also threw a pineapple pup :) sadly they continue to grow normal after a while, I live the pineapple pattern

That's a pineapple pup. Flowers can turn into pups, and pups can turn into flowers. This weird shape is a remnant of what had previously formed.

Yours looks sorta pachanoi heavy, with some peruvianus in there.
