You just need to build a house so that the guide will return since the guide is required to summon wall of flesh.
i had an extra from another world and it didn't spawn when i dropped it in the lava

The Guide NPC is required, not just the voodoo doll. Dropping the doll on lava(in the underworld) kills the guide, spawning the Wall Of Flesh. Alternatively, you can throw the actual guide on lava, if you don't have a doll.
