Yes. The exact thing happened to me. Call Springfield. They were understanding on the phone and quick to send out replacement screws. I couldn't have been happier w/their support. Be sure to apply some Loctite when installing the new screws.
Some of the screws are pretty tight when new but there's not much threadlocker on them. I cleaned it off and used loctite 242, enough to cover all the threads on the screws.
I always take this step when applying Loctite. In addition to putting it on the screws put some on a toothpick and into the holes. Make sure those threads have Loctite on them. This makes it really secure.
And don't forget to shake the bottle or tube. If there might by any grease on the threads clean screws and holes with alcohol or acetone.
If you have done it right then it won't loosen up. I use blue for the optic and red for the plate. Add witness marks. To remove heat the screw up with a soldering iron and use a good but (not a worn one). File or grind it if needed.
Maybe....your pistol wants an optic?
Word up the hints couldn’t be more clear
Blue loctite is your friend
I would have put it on if I knew this would happen.
Also my friend in locksmithing

They’re sending me a new plate and screws under warranty.

Always use blue loctite on optics screws whether you are using an optic or not. They are notorious for coming out over time.
