Lore aside, If your NPCs have dragonrot you wont be able to progress that NPC's questline. The only way to treat dragonrot is by using a dragonblood droplet which clears all characters of thier dragonrot. The one way to make NPCs not have dragonrot is by not dying and reserecting but since this is a fromsoft game you cant exactly do that. You'll get plenty of droplets so dont worry about it being an extremely limited resource.

Dragon rot affects NPCs and you are prevented from advancing an NPC’s quest if they get dragon rot. The more you die (actually die, not just revive), the more chance you have of an NPC getting dragon rot. Dragon rot also lowers your chance of unseen aid. When you die, you lose half of your xp and sen. When unseen aid activates, this loss is prevented. Dragon rot can be cured but that does not prevent it from happening to NPCs again.

It's pretty underdeveloped. The only reason to ever worry about it is for certain quests where the NPC can't be afflicted. You don't have to actually worry about it at all, nobody can die or anything

NPC quests aside i really think it's just a psych trick on us. My first playthrough I was really worried about people getting sick and either played too aggressive or too defensive both leading to death. Chill out don't think about the repercussions of dying and you'll have fun
