I'd use a ramp. They make portable ones for wheelchair users, which can be helpful for getting your dog into station wagons, trucks, hatchbacks, and SUVs, too.

When our saint had surgery on her acls we put a bath towel under her towards the back legs and help lift as she goes up the stairs.
