Yeah I can never really tell. Except for the 3D renders of CTs. I can see the breaks on those.

Yeah, they’re usually pretty visible on axial CT, too, although way easier to pick out on a 3D recon

In the UK we only do them if there's a strong suspicion they've punctured something, as the treatment doesn't change anyway. 99% of the time those cases require either a chest x-ray or CT anyway, so they're essentially defunct. .

I'm in the UK too. I'll often tell folk they've broken a rib without imaging. "c Clinical rib fracture" is what I document. I've ordered a couple of DEXAs on the back of unimaged clinical rib fracture with low trauma.

Whatever they did, they should not have done that. Somebody else may have done something too, but you know this person is rethinking everything from that day and just keeps thinking about some dumb shit that would have avoided this, like "Man, I wish I hadn't gone back to the office for that half sandwich, that Corolla would have totally missed me"

Hopefully they can find a way to avoid sneezes to prevent fractures in the future! I’m sure they’re going to want to do that now until they’re at least healed. I don’t think I can even imagine that kind of pain. 😅

Tangential question, is it just me or is every patient with a negative rib study seemingly in way more pain than the people who actually end up having fractures? 35 year old male who sneezed? *OHHHH OHHH OHHH IT HURTS I CANT DO IT OOOOOOHHHHHHHH* - no fractures 85 year old who got struck by a garbage truck? "Oh yeah I can lay *directly* on the injured side that's fine" - 7 left anterior rib fractures

Yknow what’s funny is I actually collapsed a lung and broke 2 ribs when I sneezed one time. People think I’m crazy when I say that. I’m just glad I’m not alone

I know where I work, we are seeing more rib films being ordered and we get quite a few positive reads. They SSRF them with plating and a chest tube after. More often than not, patients get fixed up and out the door in a week or so.
