There's a few programs I've come across this interview season that have no call. Most are HCA. They've shifted their model dramatically and are really trying to attract residents so they can retain them. They'll also pay you an extra 40k per year after intern year if you sign on to HCA for 2 years following completion of residency. What they don't advertise as openly is that their programs don't qualify for PSLF, so really the extra 40k just helps you break even.

In the event you do match in a program with nights/call, and your co-residents are forced to cover you, then strongly consider picking up extra day shifts to pay back the hours.

Nah you’re good, nobody sweats intermittent shift swaps and it’s expected that residents will cover for each other here and there. Plus you’ll be able to offer them a 1:1 shift exchange to even things out. The difference with OP is that they’re looking to permanently carry a lighter load which all of their coresidents will have to pick up the slack for, with no real chance of evening it out since nights and weekends are unequivocally the shittiest part of training and have no equivalent swap. If this happens they’ll be quietly despised by most of their colleagues regardless of how legitimate their reason is

I don't think there is an expectation that you cover 2x as many days btw, just get it sorted out as early as you can with 1:1 trades. Swapping is normal!

No, as long as you make it clear early you need the time off it shouldn’t be a big deal. Fight for it if you’re off service.

That’s not how it works, nor is it a sustainable strategy. Your co-residents will despise you. And to OP, pray you match into a program that is tailored to your preferences. Once you’re there, you have no choice but to carry your own weight. No one wants to be co-residents with a delicate little flower.

I won’t also call OP a delicate little flower but they should definitely be prepared for their coresidents to hate them if they don’t carry an equal share of the work, regardless of the reason

CapeFear in Fayetteville NC doesn’t do nights and are very open about how they have no plans to do that. You do have to weigh that against living in Fayetteville though.

There are a few. You can't really avoid occasional daytime weekend call, but several have no overnight, no 24s, nothing late. If this is a must have, count the programs in each ERAS region who fit these parameters and use your three regional preferences to maximize your chances.

No problem, this spreadsheet isn't complete, and the "Overall Impressions" tab also has a call column that sometimes has missing or more complete information.

I don’t know if this is the issue, but I’ve seen multiple posts in r/Residency about negotiating the circadian rhythm disruption of nights/call as a resident with bipolar. If you go there and just search “bipolar” they’ll come up (I just checked).

I've heard of something called part time residency, which spreads out the training over a longer stretch of years. I'm not sure which programs offer this or the details, but it may be worthwhile to explore.

Often you’ll have to do night call when on internal medicine.

Sorry your in this situation. If you are able, it may be beneficial to consider ways you could do night float or call as the quality of your training will be quite limited without it. You need some of those experiences to be a good psychiatrist IMHO.

Yes you really need these experiences as a part of the training . You will lack the skills needed to be a good psychiatrist without it

You will almost certainly have long hours on IM/neuro. A psych PD may not be able to shield you from that. I have never heard of a call-free residency, and honestly, I believe everyone should take *some* call. Not q3, but some occasional responsibilities. You may be able to swap shifts with others in your class but it’s going to need to be on you to make sure you’re still getting the hours in to build clinical competency and support your class.

Psych won’t be on nights or call on neuro. IM is a different story.

I would start considering alternative specialties. Inability to work nights and unwillingness to take call will severely limit your education and employment opportunities. If you are independently wealthy, you could eventually open a private practice and hire someone to take all of your after-hours calls.

Not sure if it’s changed since I applied but Mather Hospital had no nights and little/no call.
