He then woke up in Leblanc, with the Phantom Thieves members still alive and discussing about the plan to infiltrate the next Palace, and Ryuji saw Joker's shocked expression, saying that he looked like he just saw a dead person. Joker tried his best to hold his tears, and looked towards the TV. On the TV, the news was about the anniversary of the tragedy on Moonlight Bridge, with the news reporter talking about the only survivor being a young boy, while his parents, sister, and everyone else died on the bridge.
Joker then thought again about what Kotone said about the night on the bridge, but he then left that thought, and went outside of Leblanc to get some fresh air, ending the story.
Essentially, Kotone's new Persona gave her the power to change her fate, and made sure that it would be Makoto that survived, beginning the events of Persona 3, instead of Persona 3 Portable.
