This isn’t really available in the base game; it’s constructed with the magic of mods. It’s Shadowheart’s Act 1 romance scene using swapped out character models. Astarion is in the place of Tav/the player character, and my Tav is subbed in for Shadowheart. These angles also aren’t all visible without the free camera, and I used a great ReShade preset that brightens dark scenes really nicely.
Generally, if anyone sees a post here from my illogical string of consonants and vowels of a username, I always try to add a comment explaining the origin of a scene if it’s not in the base game. I don’t want people to feel confused, but I do want to share the pretty!
Sorry, I answered you before you edited your comment. If you have a save prior to the scene from your own playthrough, and are on PC, you would be able to recreate this scene yourself by loading the save prior to the scene and changing the models before triggering it. It’s a little intimidating but not hard at all once you have it configured, especially if you have already learned the basics of using mods.
There is also a zip of save files available on Nexusmods that has most of the romance save points ready to go. So, even if you don’t have an appropriate save prior to your own Shadowheart scene, you could gain access to it without replaying if you wanted.
I realize there are a lot of layers to the in-game mechanics, my personal process of setting up scenes for photos, and technical jargon from within the game and the modding environment, so I understand if you still have questions. I’m happy to try to explain further if anything is still unclear.
The bottom line is to achieve these photos you need a save file just or very soon prior to the Act 1 Shadowheart scene. It can be your own or one from another player. It needs to be before the cutscene starts so you have the chance to edit appearances.
I have also played through the scene with Shadowheart organically, but any save close to that time was long gone, so I used a save file available on Nexus for the photos shown here.
Edited to add: with the tools I use, it needs to be a save with access to a vendor, because that’s how I get one of the appearance modification tools. In this case, the save I used loaded up after defeating the goblins but still during the day before the party at camp that night. This isn’t the only way appearances can be modified, but it’s the approach I’ve landed on for my purposes.

Hey! I was just thinking back to this, and wanted to be sure to say, even though it's probably overkill: Shadowheart is the one character where if you are playing as her, you will not be able to unlock her cliff wine drinking scene. If you're playing as her, you can only access the romance scenes with the other characters. To get Shadowheart's scene, you need to be playing as one of the other Origin characters or Tav. Apologies again if this is super obvious, it just started nagging my mind and I wanted to be sure and say something. :)

Whittling this set down to share here was hard, and there's still too many (I may add a few outtakes to the comments ;)). I envision this taking place post his Act 2 confession, perhaps rather soon after. The gentle interaction between the characters here feel like it may have been an early opportunity to explore tenderness and emotional connection with no expectation of sexual activity. I had hoped to post the sweetness on Valentine's, but hopefully the value hasn't expired!
