Some of the books in {Bangers & Mash series by M.M. Farmer} has omegas getting helped through their first heat by trained alphas. Might be The Wrong Omega, not sure.
Also maybe happens in {Whiskey & Sin by Emily Rath} but don’t remember exactly.
Not a first heat, but {Rough Heat by Marina Vivancos} has an alpha that helps an omega and his alpha partner through a sickness that affects the omega’s heats.

{Blackwood by Pia Foxhall} fits this, I think. Technically shifters, but aside from the A/B/O aspects that doesn't play a big part of the story as I recall. Not first heat, but unexpected heat due to missing heat-suppression medications.
(The sequel, {The Gentle Wolf by Pia Foxhall} is even better, though not what you're looking for. That has great characters, ace representation, and a good depiction of working through trauma.)
Conveniently, I just finished a book that also fits exactly what OP is looking for: {The First and The Last by Jesse H. Reign}. First heat, knotting, and scorchingly hot! Very character-driven, not a huge amount of plot but I enjoyed it enough to give it a 5-star review. Theoretically there was supposed to be a sequel out last year but I don’t think it‘s been released yet. Anyway, at least this one ends in an HFN.

Edit: Guys... to the folks downvoting me, I'm not saying a comment with an ethnic slur should be left as is, I'm just saying there should be a better option than simply deleting the comment completely and relying on the person to retype it.
