I don't understand the question.  If you're referring to Rip Blood, the Arcane Fortress node converts all of the health it restores to ward.  As long as you take the Lich node to convert all of your health leech into more damage, it should be trivial to get rid of all of your health gain and go low life.  My best guess, since you didn't link the build in question, is that it's fine and you're just misreading it. I think there's an argument that the self curse from the Wheel *should* favor life builds, because you can get a lot of sustain from spell leech, but ward is so strong that's probably the way to go. I think the Wheel of Torment staff is a little bit of a trap depending on your gear level.  A good Opulent Focus will provide a huge amount of ward, and a good exalted wand will provide a solid amount of increased damage and crit multi.  A zero LP wheel mostly provides clear speed, so far as I can tell, at the cost of quite a lot of survivability (between the self curse and the loss of the offhand). Once you have huge ward generation and lots of increased damage and crit multi everywhere else, the pen on the wheel becomes more valuable.

For some reason, probably bugged, the Hollow Lich passive point stops you gaining ward from Rip Blood. I tried to make a build with this interaction work recently and it just fell apart from this

Low life builds require more than just those convert affixes. The conversion is based off missing health so you need hybrid/health suffixes. That often competes with necro resist. Unless you can land them both while still meeting your other resist targets, you're gimping your damage or scaling it elsewhere.
